Fasiyal Sinir Hastalıklarında Tedavi Yaklaşımları

Abstract

Fasiyal sinir paralizisi enfeksiyöz, nörolojik, konjenital, neoplastik, travmatik, sistemik ve iatrojenik nedenlerle oluşabilir. En sık görüleni idiyopatik fasiyal paralizi(Bell’s paralizisi)’dir. İdiyopatik fasiyal sinir paralizisine sıklıkla konservatif yaklaşılır ancak eğer paralizi kompletse ve elektriksel testler gerekliliği işaret ederse cerrahi yaklaşım önerilebilir. Burada fasiyal sinir cerrahisinde ki temel amaç dekompresyon için sinirin etrafındaki kemik dokuları uzaklaştırmaktır. Bununla birlikte özellikle travmatik ya da tümöral kaynaklı paralizi durumlarında diğer cerrahi seçenekler uygulanabilir. Bu cerrahi yaklaşımlar direk onarım, kablo sinir greftlemesi, sinir crossover tekniklerini içermektedir. Bu derleme çeşitli nedenlere bağlı periferik sinir paralizilerinde cerrahi tedavi seçeneklerinden bahsetmektedir.
