HEMŞİRELERİN HASTA DEVİR TESLİM SÜRECİ

Öz

Hastaya verilen önlenebilir zararlar, birçok ülkede hastalık yükünün önde gelen nedenlerinden biridir. Bu nedenle, hasta güvenliği küresel olarak sağlık hizmetleri için önemli bir konudur. Hasta güvenliğinin sağlanması için hedefler oluşturularak iyileştirmeler yapılmaktadır. Ulusal ve uluslararası olarak hasta güvenliğinin sağlanmasında, bakımın kalitesinin arttırılmasında hasta devir teslim esnasındaki iletişimin önemi vurgulanmaktadır. Hasta devir teslimi bilgileri açık, anlaşılır ve gerekli olmalıdır. Hastalara sürekli bakım sunan hemşirelerin rutin uygulamalarının anahtarı hasta devir teslimidir. Hasta devir teslimi bir hemşirenin hastayla ilgili bilgileri, sorumluluğu ve karar verme sürecini diğer hemşireye devretmesidir. Hasta devir tesliminin başarılı olması için hemşireler uygun bir iletişim aracı kullanarak standardizasyon sağlamalıdır. Hasta devir teslimi yatak başında, hasta katılımıyla gerçekleştirilmelidir. Standardizasyon içerisinde gerçekleştirilen devir teslimin hasta bakımının kalitesini, hasta memnuniyetinin ve hastanın hemşireye güvenini arttırdığı bilinmektedir. Bu derlemenin amacı hasta güvenliği ve hemşirelik bakımı için önemli olan hasta devir teslim sürecinin öneminin vurgulanması ve güncel bilgilerin paylaşılmasını sağlayarak hasta devir tesliminin kalitesinin arttırılmasını sağlamaktır.

Öz

In many nations, one of the main causes of the burden of disease is avoidable damage to the patient. Patient safety is therefore a crucial concern for the global healthcare system. Goals that guarantee patient safety lead to improvements. Nationally and internationally, emphasis is placed on the importance of communication in ensuring patient safety and improving the quality of care during patient handover. Patient handover information must be clear, understandable and necessary. For nurses who provide patients continuous care, patient handover is essential to daily practice. Patient handover is the process by which one nurse gives another nurse access to information, responsibility, and decision-making regarding the patient. Nurses must use a suitable communication tool to achieve standardization in order for patient handover to be successful. Patient satisfaction, patient's trust in the nurse and the quality of patient care are known to rise when handover is done within the framework of standardization.At the patient's bedside, patient handover should take place with their involvement. In addition to highlighting the value of the patient handover procedure for nursing care and patient safety, this evaluation aims to improve patient handover quality by disseminating current knowledge.
